NP Knowledge of Death Pronouncement in Georgia Survey

Dear Nurse Practitioner:

Please assist us in eliminating Nurse Practitioner barriers to practice in Georgia by completing this short 11 question survey. We are nurse practitioner graduate students from Valdosta State University in Valdosta, Georgia engaging in a research project. The objective of this project is to evaluate nurse practitioners’ understanding of current policy regarding pronouncementof death in Georgia hospitals. We hope that the results of this study will assist us in changing barriers to practice in Georgia.

This questionnaire will take approximately five minutes of your time to complete. There is no compensation for completing this survey, nor is there any risk. Survey participation will be voluntary and responses will be anonymous and confidential. You may refuse to participate at any time. If you choose to participate, please answer all questions as honestly as possible, and complete the survey in its entirety.

Enclosed with this letter is a brief survey of eleven questions that asks about your current understanding of policy regarding pronouncement of death in Georgia. We are asking that nurse practitioners from the state of Georgia with a current and valid license complete the questionnaire and return it in the postage-paid envelope as soon as possible.

Thank you for taking time to assist us in our educational endeavors. We hope that the data obtained in this survey will help to overcome nurse practitioner barriers to practice in Georgia.
